Menopause Symptoms Checklist

When your ovaries stop producing eggs the hormone levels in your body decline. Hormones affect every cell in our body so when they are fluctuating leading up to the menopause (perimenopause), this can cause symptoms all over our body.

Our tracking tool is an effective and straightforward way to measure and track your symptoms.

The main areas that are measured are psychological, physical, vasomotor and vaginal/bladder symptoms.

This checklist is very helpful for you to complete before your consultation. Done regularly, it can help monitor (track) how your symptoms change with time or with treatments, especially when taking HRT (Hormone Replacement Therapy).

Enter your email address after the form below if you would like your completed checklist emailed back to you. It will be date stamped and you can complete it whenever you like so you can track your progress.
